Eligibility and Clearance  

In an attempt to streamline the athletic clearance process, Sossaman Middle School has gone paperless.  You will now complete the athletic clearance packet on Aktivate.  All forms can be viewed, signed, and uploaded electronically. It is strongly recommended that all documents be submitted electronically prior to the break that precedes your child's sport season (or prior to the school year beginning if participating in a first-quarter sport). Once school resumes after break, only forms submitted prior to 1:45 will be cleared for that day.

1.       Create an account on Aktivate (find instructions here). This is where you will sign all the release forms and submit your student’s physical and concussion test results.

a.       You can select any sport your child might be interested in. This does not mean your child is signed up for that sport, it just gives the coaches access to your student's paperwork if they decide to go to those tryouts.

2.       Complete the concussion test. You can find more information about this here. Submit the completion certificate to Aktivate when your student is done.

3.       Complete the Physical form. Only physicals completed on the current school years AIA athletic clearance page will be accepted. Submit the completed physical to Aktivate. 

a. Please make sure to submit all five pages. Double check that there are no signatures missing and that the doctor's office information is filled out. If your doctor indicated that your child needs to see any specialists, we will need the paperwork from that visit as well.

4.       When your Aktivate account has a green bar across the top, this means you have completed all the steps, and your student is cleared to participate in sports activities for the school year. If your account is showing red, but you believe you have completed everything, please contact the school to find out more.

Our school is a member of the East Valley Conference. The EVC website provides information including scores, directions to the schools, and playoff brackets.
